چکیده
Perioperative chemotherapy (PCT) is the standard of care for locally advanced gastric cancer (LAGC) in western countries. However, less than 60% patients complete adjuvant part treatment due to postoperative complications, toxicity and disease progression (DP). This study aim evaluate if preoperative chemosensitivity (POCS) associated with improved overall survival (OS) free (DFS). Retrospective, multicentric 175 consecutive LGAC treated PCT curative resection 7 Portuguese hospitals from 2016 2021. We defined POCS as sensitive (partial or pathological responses) refractory (no response progression). One hundred seventy-five medical records were reviewed. Nineteen excluded because surgery was not performed DP. The median age 66 years, 114 (73.1%) male. All ECOG 0-1. Regarding neoadjuvant chemotherapy, 117 (75%) received FLOT, 16 (10.3%) ECF, 11(7.1%) FOLFOX, 9 (5.7%) EOX, 3 (1.9%) EOF. Forty-nine (31.4%) classified 107 (68.6%) (94 had partial 13 response). Fifteen did toxicity. At a follow-up 27 months, resulted statistically significant 51% reduction risk death (HR 0.49; 95% CI, 0.24-0.71; p=.001). Median OS 31 months (95% 17.5-44.5) group reached (p < .001). Mean DFS 22 16.3-27.7) death, 0.40; 0.25-0.65; p Despite limitations our sample size, an LAGC perioperative surgery. These results need be validated more real-life evidence based studies may help inform future personalise therapy.
